First Session Policies & Paperwork

You can request information or set up an initial appointment by filling out our simple contact form, or you can email us. We will reach out to you and send paperwork that you will need to review and complete before your first appointment.

For your first appointment, you will arrive with all paperwork already printed, completed, and signed (if this is an issue, please let us know). You may wait in the lobby for your therapist to come get you. Payment will be collected prior to your session, or we can use the card you documented on your card payment authorization form. You can find more information in the intake paperwork. Your therapist will go over completed paperwork during your first session.
